Confidentiality and Privacy

 

The Ontario Government has passed the Personal Health Information Protection Act, 2003. The goals of the Act are: 

  • to establish rules for the collection, use and disclosure of personal health information.

  • to provide the right to access your health record and the right to make corrections or amendments to personal health information.

  • to provide an independent review and resolution of complaints that relate to your personal health information.

    Collecting Personal Information

    Personal health information is collected from each resident, as necessary for the purpose of:

  • Managing each resident's personal care;

  • The overall management of the Home's health delivery system;

  • Resident safety and quality improvement;

  • Legal and regulatory requirements, e.g. the Ministry of Health & Long Term Care.

  • St. Patrick's Home limits the collection of personal health information to that which is necessary for the purposes identified.

    Using Personal Information

    We use your personal health information only for the purposes we have outlined;

  • Your consent  will be requested by the Home if your personal health information is needed or wanted for purposes other than those already identified;

  • St. Patrick's Home strives to ensure that your personal health information is as accurate, complete, and up-to-date as possible;

  • We safeguard your personal health information;

  • The Home has in place strict policies regarding the confidentiality of each resident's personal health information and staff receive regular training regarding these policies;

  • The Home retains personal health information for a period of 20 years upon discharge of a resident, or for a period of 7 years following the death of a resident in the Home, in compliance with legislation or Ministry guidelines;

  • St. Patrick's Home has in place a policy which describes the method for destruction of personal health information following the completion of the prescribed amount of time for retention.

    Disclosing Personal Information

  • We disclose your personal health information only for the purposes that have been identified;

  • Your consent is required to disclose any additional personal health information;

  • We will not disclose your personal health information if you have withheld or withdrawn your consent
