The Social Services Department is
located on the Ground Floor, past the Chapel. The Manager of
Resident and Family Services is available
from Monday to Friday, 8:30 am to 4:30 pm, for the resident and his/her
family to discuss concerns and difficulties experienced while adjusting to
a new life in the Home.
This same service is available on an ongoing basis.
A Care Review will be scheduled with the resident, family and the
interdisciplinary team approximately 4-6 weeks following admission to
ensure that the resident's needs are being met and to discuss areas of
concern. Subsequently, a Care Review will be scheduled yearly for the same
purpose.
If a resident requires or desires a discharge, the facility will endeavor
to identify appropriate placement options in collaboration with the
resident/authorized person. The facility will liaise with the Community
Care Access Centre to assist the resident in coordinating community-based
services. |
