A Care Review will be scheduled with the new resident and family and the interdisciplinary team approximately 4-6 weeks following admission to ensure
that the resident's needs are being met and to discuss areas of concern. |

|
|
As Your Needs Change
Rooms
must be appropriate for the care requirements of
the resident. At some point, it may be
necessary to have some personal belongings or
furniture removed to allow the staff to have adequate
access to the resident.
Nursing staff will report any concerns in this regard to the resident and/or family.
Occasionally, it may be necessary for residents to move to a room
or unit more appropriate to their needs.
This is
most important when the staff need to use assistive devices, such as lifts or specialized chairs, to give the care required. |

New Residents
Group
All new family
members and residents, if they are able, are invited to attend the “New
Residents Group”. This three week series is an opportunity for you to
meet the managers of the different services of the Home and to ask
questions or to express any concerns or worries. It is organized by the
Social Worker who met you on the day of your admission. It is an
informal meeting, with coffee and tea served, and gives you an
opportunity to meet others who have recently moved to St. Pat’s. |

Removal of Belongings on Discharge from the Home
While not a great topic when you're thinking about coming to live at St. Pat's, it is necessary that you be aware of what needs to be done once you are no longer here. Upon discharge from the Home or death, arrangements must be made by your lawfully authorized substitute to collect your personal possessions within 24 hours.
The Home can provide some assistance in packing.
